The Alameda Health System Palliative Care Team offers consultations for inpatients in all of our acute care hospitals (Alameda, Highland, and San Leandro), and for outpatients at the Highland Wellness Center.
To request a consultation for a hospitalized patient, please call the hospital where the patient is hospitalized, and ask to be connected to the Palliative Care Team.
